About the Role

We’re seeking an experienced Clinical Care Coordinator to play a pivotal role in shaping safe, high-quality, person-centred care within our Residential Aged Care community.

Reporting to the Clinical Care Manager, the Clinical Care Coordinator provides clinical leadership, oversight, and coordination across our residential care services. You’ll partner closely with nurses, care staff, allied health professionals, residents, and families to ensure care is safe, compliant, and genuinely person-centred.
This is a hands-on leadership role where your clinical expertise, judgement, and people skills will directly improve resident outcomes and staff capability.
What you’ll be responsible for
- Providing expert clinical leadership and guidance across residential care
- Conducting comprehensive assessments and overseeing high-quality care plans
- Supporting nurses and care staff through coaching, supervision, and development
- Managing complex clinical issues including wounds, falls, behaviours, and deterioration
- Ensuring compliance with the Aged Care Quality Standards and legislation
- Leading incident management, audits, and continuous improvement initiatives
- Building strong relationships with families, GPs, allied health, and external stakeholders
- Promoting a positive, safe, and values-led workplace culture aligned to SONA®
Skills And Experiences
About You
You’re a confident clinical leader who brings both capability and compassion. You enjoy mentoring others, navigating complexity, and balancing governance with genuine human connection.
You’ll bring:
- Registered Nurse qualification with current AHPRA registration
- Minimum 3 years’ experience in a senior clinical role within aged care
- Strong clinical assessment, care planning, and documentation skills
- Sound knowledge of the Aged Care Quality Standards
- Proven leadership, communication, and problem-solving capability
- Strong IT skills and experience with care management systems
Postgraduate qualifications and funding experience are highly regarded.
Role Requirements
- Satisfactory police check (or willingness to obtain through BallyCara)
- Right to work in Australia
- COVID and Influenza vaccinations highly recommended
