BallyCara is looking for an experienced Registered Nurse to join our Support at Home team, supporting clients between Moreton Bay and North Brisbane Regions on a full-time 12-month fixed term contract.
This role plays a key part in onboarding new clients and managing a clinical caseload, helping older Australians live independently and confidently at home.

The Opportunity
As a Care Partner – RN, you will play a central role in welcoming new participants into the Support at Home program, completing assessments, and coordinating services to help them achieve their health and independence goals.
You will also manage a caseload of participants, providing clinical oversight and coordinating services that support wellness, reablement and restorative care.
In this role you will:
- Lead the onboarding and assessment of new Support at Home participants
- Develop person-centred care plans and goal-based support plans
- Manage a clinical caseload of participants living at home
- Coordinate services across nursing, allied health and support providers
- Provide clinical guidance and oversight to support safe, high-quality care
- Build strong relationships with clients, families, GPs and service providers
About You
You are a confident and compassionate Registered Nurse who enjoys working autonomously while collaborating with a multidisciplinary team.
You thrive in a community-based role where strong relationships and proactive care coordination make a real difference.
You will bring:
- Current AHPRA registration as a Registered Nurse
- Experience in aged care, community care or case management
- Strong skills in clinical assessment, care planning and coordination
- Experience supporting wellness, reablement or restorative care models
- Excellent communication and relationship-building skills
Role Requirements
- Satisfactory police check (or willingness to obtain through BallyCara)
- Current First Aid & CPR
- Current Driver’s License
- Right to work in Australia
- COVID and Influenza vaccinations highly recommended
