The Role

We are seeking a compassionate and proactive Engagement Partner (Registered Nurse) to join our Support at Home program. In this role, you will:

  • Engage with local hospital networks public / private discharge planners PHNs, GPs assessment agencies to build strong links & referral pathways for new & existing clients.
  • Creation of new business and sustain current clients by accessing additional services / programs.
  • Engage with clients, families, and referral networks to attract, retain, and optimise home care services.
  • Collaborate with Care Partners and clinical teams to assess client needs, promote restorative care, and ensure timely transitions between care levels.
  • Contribute to reporting, continuous improvement, and operational performance through accurate record-keeping and stakeholder engagement.


Skills And Experiences

Key Requirements

  • Current AHPRA registration as a Registered Nurse.
  • Experience in aged care, community health, or a related sector.
  • Knowledge of the Support at Home program and the Australian aged care system.
  • Strong interpersonal, communication, and digital literacy skills.

Desirable

  • Qualifications in aged care, community services, or a related field.
  • Experience with restorative or reablement programs.
  • Understanding of client retention strategies and business development in aged care.
